Hello. Welcome to our OPTOUT page. All the information here is taken from trusted sites and we have tried to make the process as easy as possible for you.
We recommend that you OPT OUT AS SOON AS POSSIBLE. Current deadline is August 25th but that is not confirmed by the Dept. of Health so we just say "urgently".
There are TWO STEPS to take. Luckily they do not take a long time but it's worth taking careful time to ensure you succeed. Approx. 10 - 15mins to do both steps.
PLEASE NOTE: This will NOT affect your access to NHS Treatments. And you can Opt-In again at any time. We simply recommend you opt-out because the ethics of this cynical move by the Dept. of Helath and NHS Digital should not be allowed to proceed and public opting out will help to make that clear.
STEP ONE: Type 1 Opt Out (GP Surgery)
This tells your GP Practice that you do not wish your personal patient information leaving the GP Practice. By filling out the TYPE 1 Objection a code will be placed on your patient records and will not be automatically transferred on September 1st.
It is a very simple two sided form with name, address details and a tickbox to OptOut.
(Don't worry about NHS NUmber if you don't know it).
Select the "I DO NOT WISH" option.
* Download either the PDF VERSION or WORD Version(allows you to do it online)
*Either print, write in details and take/send to your GP Surgery
OR
*Fill in online, add electronic signnature, email to Practice Manager requesting a reply.
IMPORTANT SECOND STEP
We recently discovered that Somerset NHS Foundation Trust wants to SELL its data (patient personal information) to a company called Sensyne.
